1.What two primary items determine the class of operation of an amplifier?

2. What amplifier class of operation is the most inefficient but has the least distortion?
 
1) amplifier circuit configuration & Bias
2) Class A

Second answer is correct and first answer is a bit vague about 50% correct!
 
A class A amplifier has the transistor or tube always on no switching and the power suply is always at full potential. A class AB amplifier oprates as a class A amplifier up to a certin point and then switches to a class b after. A sliding class has is a servo controlled power suply. And then there is the switching power suply amplifiers d g h...

A class a amplifier will actually run cooler at full tilt.

It depends on what the amp is used for what one class of amp excels in may be very poor in another application. It different class amps have different sequence times for the signal to go from input to output.

But there are two basics that govern that nomenclature! What are they?
 
If the amp always uses polite language in the company of women, children, and strangers, it has uncommon class.

If, it is likely to drop blue chatter at any given moment, it is low class.
