Ideally with a local lawyer, as restrictions and requirements vary by location and you want to be protected, but barring that, a template like those at the following links will be a starting point:

Just write down whatever it is you and the customer agreed to, and what consideration is being paid to make that happen. A contract specific to what you are doing is the best document. Then, answer a few questions that both of you might have: "what if one of us has to cancel or make a change?" What if there's some kind if equipment failure that intrudes on the event? What if some natural disaster or other condition prevents travel to the gig or the gig itself can't happen safely or legally?
You don't have to have all the answers and an iron clad contract. Just the details of what is being exchanged for the amount of consideration (payment) agreed.
All of the other stuff just describes how much risk or liability each of you is willing to assume in a given circumstance. Clause that you don't have included won't necessarily hurt you because there is a body of law and precedent to deal with that.
